Problem with Outlook 2007 custom form

We have a custom form that was made awhile back with an older version of MS Outlook, when we try to use it with OL 2007, it seems to cause odd issues like sending our new logo as an attachment instead of a picture within the signature...
it also seems to affect sending attachments when forwarding...
(the original attachment is not sent when choosing to forward an E-mail that has attachments- warning- "attachments could not be found")
Does anyone have any real experience creating working custom forms from within OL 2007?
I'd appreciate it...

For the Signature....Edit the sig - right click and select "Insert Image"

Thanks but thats what we did, and even redoing it doesnt change how it works...
I'm assuming the form needs to be reworked from scratch...
Does anyone have experience with this?
Its within a network environment, and its an office pulling from a custom form from awhile back (old versions of Outlook)
