Portable multimedia handheld device

I am searching for a portable multimedia device that can play music and videos.

It should have:

- A large storage capacity (at least 4 GB), perhaps in the form of a mini hard drive (like an iPod).
- A screen with enough color support for videos, big enough to read subtitles on.
- Play various multimedia formats (like mp3's and avi, divx, xvid, ogg, etc. IT NEEDS TO SUPPORT SUBTITLES!).
- A Headphone input
- Have enough battery life to last at least 8 hours on a full charge.
- Removable media of some sort.
- Concealable shape and size (to fit in a pocket).

It would also be great if it had a carrying sleeve too.

Is there such a device?
