Knowledge base article from MS describes this problem and a work around for it.
OrgChart: Cannot Insert OPX File into Document Under Windows NT 4.0 or Windows 2000

Quote:
WORKAROUND
Insert with Organization Chart
To insert an .opx file into a presentation without using the Insert menu, you can start Organization Chart, open the file, copy it, and then paste it into your document.

NOTE: The following steps assume that your document is already open.
Click Start, and click Run.
Click Browse and navigate to the C:\Program Files\Common Files\Microsoft Shared\Orgchart folder and select Orgchart.exe.
Click Open, and then click OK. This will start Organization Chart.
When the Open Chart dialog box appears, select the .opx file you wish to use from the list, and then click Open.
After the .opx file loads, press CTRL+A. This selects all the items in the orgchart.
On the Edit menu, click Copy.
Switch to your document.
On the Edit menu, click Paste.
The Organizational Chart now appears in your document.

I would have thought that this is possibly due to the fact that it just isn't installed. Go to add/remove programs, select office, click change, select office tools and choose MS Organisation Chart, then run from my computer or network depending on your setup and then click update. Then go back and try and insert an object and it should be there.

But of course if you've already done this, I'm talking rubbish!

Cheers, Steve.
