Help with creating a Thank you/Award form.

I am having a hard time trying to create a way to create a form that will email someone with an award/thank you.

At my work, we are trying to make it possible for staff members to recognize each other for doing a good job. The goal is to have a form that a member fills out (IE: Their name, Email, a few words to describe what they did, the recipient name and email.) so that they can send a thank you. I am looking for a way so it will email the thank you and it will look as though it is on a certificate. That way, if they want to print it out, they can.

Here is what I have successfully created. You can put all your information in such as Your Name, Recipient's Name, Recipient's email address, and a place to write a short thank you. The only problem is that it only emails it as a text. I am having a hard time finding a way to make it appear as a certificate in the recipient's email.

I have been looking on google for information, but the only thing I have found was a similar form that will only email text. I do not know if I need to do this in PHP or just HTML.

create the basic image without the text filled in, then use php to load the background image and put the text into the image on the fly (final finished product is just a regular bmp or jpg that they can print!)
