Survey Code?

Hey guys,

New to the site and the codeing end of this. I been doing some research and have been trying to figure out if it is possible to get some barebone's really simple code for an online survey. This survey would go on our website and the only one person needs the results for this. Is it possible to get some really easy code (not sure if it would be html or php or whatever) and just have the results get emailed to one person?

In the for what its worth catagory - there are sites that will let you setup surveys - and then you can link to them on your site....I'll see if I can find one later today.
