Looking for Script

I'm looking for a very basic script...

It needs to display entries... Sort of like a guestbook. It needs to include a persons name, a date/time and their entry. Very very simple. A simple line should divide each entry. The only other feature I'd prefer is the ability to catch the person's IP address for each message posted.

That is really all I need, and too much more is definately not good. It has to be simple, and very streamlined. No graphics outside of emoticons, etc. Oh yea - it has to be PHP, as well.

Any suggestions?
